Outpatient Mental Health Clinic (OMHC) Referral Form

Please complete the form below to refer a client, student, or family member for outpatient mental health services at K Hawk Solutions.Our OMHC provides compassionate, evidence-based care for children, adolescents, and adults through individual, family, and group therapy, psychiatric evaluations, and medication management.

Once your referral is received, our clinical intake team will review the information and contact the client (or parent/guardian) within 3–5 business days to complete the intake process.
